Connellsville, PA · Request an appointment
Request an appointment.
Send the form at the bottom of this page and a real person calls you back, usually within two business hours, with times that fit your actual week. No referral needed to start, at either clinic.
What happens after you send this.
Four steps, in order, with nothing mysterious in the middle.
- 01Your request goes to the front desk at the clinic you picked, not to an inbox nobody reads.
- 02A real person calls you back, usually within two business hours during clinic hours, and the same day if you send it in the evening.
- 03We find a time that fits your week at whichever clinic is closer, Connellsville or Scottdale, including the Tuesday and Thursday evening hours.
- 04If you want your insurance checked first, we do that free and call you with the numbers in plain English before you owe anyone anything.
If you would rather send a form at ten at night than call during business hours, that is not avoidance. It is how most people book anything now, and the callback exists so the awkward part is on us.
Nobody gets put on a waiting list without being told, and nobody gets sold anything on that phone call. It is a scheduling call: what hurts, how soon you want in, which clinic is closer to you. If you would rather skip the form entirely, call (724) 628-7288 ⟶ for Connellsville or (724) 887-4181 ⟶ for Scottdale and we will do all of this out loud instead.
What to have ready.
Short list. Nothing on it is a reason to delay the call.
- Your insurance card, if you have one. If you do not have insurance, say so on the form. There is a self-pay rate and we quote it before you book, not after.
- Any imaging you already have. An X-ray or MRI report is useful if it exists. It is not required to start, and there is no reason to go get one first.
- Clothes you can move in. Shorts for a knee or hip, a shirt you can move your shoulder in. You will be tested and treated the same visit, so dress for it.
- One sentence about what you cannot do right now. The stairs, the top shelf, the first ten steps out of bed. That sentence is more useful to us than a diagnosis.
Plan your day around the visit itself: the evaluation runs about 90 minutes, and follow-up sessions are typically 60. That is the whole reason the first visit is long. Your licensed physical therapist has to test enough to find the actual cause, and hands-on treatment starts in that same appointment rather than at some visit down the road.
Do I need a referral to request an appointment?
No. Pennsylvania’s Direct Access law allows you to receive physical therapy without a physician’s prescription, which means the appointment you are requesting right now does not wait on anyone else’s schedule. Our old site put it in four words and they still hold up: no referral, no problem.
Two honest footnotes. Some insurance plans still want a referral on file for reimbursement, and if yours is one of them we will spot it during your free benefits check and help you get it, before it becomes a billing surprise. And if something we test at your evaluation belongs with a physician instead of a physical therapist, we will tell you that out loud and help you get there. Direct access means you can start here. It does not mean we pretend to be the only stop.
Waiting for a referral usually costs more than the appointment does. A shoulder you have stopped reaching overhead with quietly teaches the rest of your body to compensate, and by the time anyone calls, the compensation has become its own problem to unwind. Earlier is easier, almost every time we have seen it.

The people who end up filling this out are rarely in the worst pain of their life. They’re just done working around it, done picking the errand that doesn’t require the stairs. If that’s the stage you’re at, this is the part where you stop researching and start.
Consider it filed.
