Connellsville, PA · Patient forms
Patient forms.
Two sets of paperwork exist here: the intake forms every new patient fills in, and the fitness membership forms. Neither one is posted as a download on this site, so here is exactly how to get them.
Patient forms for your first visit in Connellsville.
New patients fill in an intake packet before the evaluation starts: who you are, what happened, what hurts, what you have already tried, your medical history, your insurance details, and the consent and privacy paperwork the law requires us to give you.
- New patient intake paperwork. The full packet, filled in once, before your first appointment.
We don’t post it as a download here, and we would rather hand you the current version than let you print an old one. Call the clinic and we will get a copy to you ahead of time, or ask at the front desk when you arrive. Either way it takes a few minutes, not an afternoon.
If you’re the sort of person who’d rather have the paperwork done before you walk through the door, you and this clinic are going to get along. It buys back the front of your evaluation, which is the part you actually came for.
Fitness forms.
The fitness side of the building has its own paperwork. Two forms, both by name:
- Fitness Membership Contract. The membership agreement itself.
- Fitness Membership Take-Ins. The intake sheet that goes with it.
These forms exist and we hold them at the front desk. What we won’t do is describe a program on this page that we haven’t written up properly yet. If you want to know what the fitness membership includes, what it costs, or who it’s for, ask the front desk at either clinic and you’ll get a straight answer from someone who runs it, not a paragraph from a website.
How long does the paperwork take?
Arrive five minutes early and you can finish the forms on the spot. Nobody is going to send you home over a blank line, and nobody is going to hand you a clipboard and then leave you in a waiting room for half an hour.
The evaluation itself runs about 90 minutes. That is the whole reason to get the paperwork out of the way first: it is a long, hands-on appointment with a licensed physical therapist who tests rather than guesses, and you want all of it.
Bring your photo ID and your insurance card. If you’re paying out of pocket, say so when you call and we will tell you the number before you come in, in writing, the way the law says we should. You do not need a referral to book. Pennsylvania allows direct access.
Paperwork is a bad reason to put off a phone call. The knee doesn’t pause while you look for a pen. It settles in, quietly teaches you to favor the other leg, and by the time most people ring us the limp has become a habit that also needs undoing.
Book the evaluation. We will have the forms ready when you get here.
Put it in writing.
Six short questions. A real person calls you back, usually within two business hours, with times that fit your week.
The people who end up filling this out are rarely in the worst pain of their life. They’re just done working around it, done picking the errand that doesn’t require the stairs. If that’s the stage you’re at, this is the part where you stop researching and start.
Consider it filed.
