Connellsville and Scottdale, PA · Contact
Two clinics, one phone call away.
Two fully staffed clinics, one practice, one standard of care. Every address, phone number, fax line and hour we keep is on this page, with a map to each door. Call whichever one is closer.
Both clinics: Connellsville and Scottdale.
Neither of these is the main office. Connellsville and Scottdale each run a full clinical team, the same hours, and the same one-on-one format, every visit.

Which clinic should I go to?
Honestly? Whichever one is closer to you. We are not going to invent a difference between them. Both clinics run the same evaluation, the same four phases, the same one-on-one hour with a licensed physical therapist, and the same hours of operation. If Connellsville is on your way home and Scottdale is not, that settles it.
The one thing worth calling about is the schedule. Each clinic keeps its own appointment book, so a Tuesday evening slot may open sooner at one than the other. Tell whoever answers what times actually work in your week, and they will tell you which clinic can hold that time. If the other one fits your week better, say so and we will move it.
If you are reading a contact page carefully before you dial, comparing two addresses instead of taking the first one you saw, that is not indecision. That is the same care you are about to ask us to take with your shoulder.
Patients come to us from Connellsville, Scottdale and Mt. Pleasant, and plenty of them have used both clinics across different injuries. Your records follow you either way, because this is one practice, not two.
What should I have ready before I call?
Have three things within reach and the call takes about five minutes. Your insurance card, so we can read the plan back to you. A rough timeline: when this started, and whether it has changed since. And one honest sentence about what you cannot do right now, the stairs, the top shelf, the second half of a shift. That last one is the part that shapes your plan.
You do not need a referral. Pennsylvania allows direct access to physical therapy, so you can book an evaluation without seeing anyone else first. If your particular plan requires a referral for reimbursement, the free benefits check will catch it and we will help you get one before your first visit.
The benefits check is exactly what it sounds like: we call your plan for you, then tell you your copay, where your deductible stands, and whether there is a visit limit, before you owe anyone anything. If you are self-pay, ask for the flat rate and a Good Faith Estimate. You get both in writing.
The call that gets postponed is rarely postponed once. It slides to the next quiet week, then to after the holidays, and by then the side you have been guarding has quietly taught the other side to do its work, which is a second thing to fix.
“Best physical therapy facility! I’ve been to others over the years, but King’s worked wonders for me.”Bobbi R. · Patient review
Pick the closer door. We will handle the rest.
Put it in writing.
Six short questions. A real person calls you back, usually within two business hours, with times that fit your week.
The people who end up filling this out are rarely in the worst pain of their life. They’re just done working around it, done picking the errand that doesn’t require the stairs. If that’s the stage you’re at, this is the part where you stop researching and start.
Consider it filed.
